I give permission for my Child to take part in the "Evolution of dance " 2022 with DWA Performing Arts. I understand that my child needs to attend a rehearsal at the theatre that will be an extra cost of £15 Payable on the day. My Child is able to perform in both shows 30th Jan and 6th Feb 2022. I give permission for my child to be photographed and Videoed during the rehearsal and performances for promotional purposes.
